Patient Information

All initial visits with our physicians or physician assistant are consultations. This means your visit will be to meet the physician or physician assistant and discuss your health problems and symptoms. No procedures are performed on the first visit.

We strive to make your appointment go as smooth as possible; here are some things that you can do to help:

  • New Patients should arrive fifteen minutes prior to your scheduled appointment time. Remember, if you arrive fifteen minutes after your scheduled time, your appointment will be cancelled.
  • Please bring your insurance card(s); completed Adult Registration and current medication list.
  • Write down a few of the most pressing questions or concerns regarding your gastrointestinal symptoms, so that we may address them for you.
  • Please be prepared to sign your Privacy Practices Acknowledgement and Office Policy Agreement forms,
  • All co-pays required by your insurance, will be collected at the time of your first visit. In addition to cash and personal checks, we also accept Visa, MasterCard, and Discover card for your convenience.
  • If you are an established patient of DHC and have any changes in insurance, address, phone number, etc. Please give our staff the new information so that we may have the most correct and most current information in your chart.
  • Prescription refill of current medications available by contacting your physicians nurse. When calling please leave your name and number, name of your current medication to be refilled and the pharmacy name and number. Our staff will call the refill to the pharmacy before 5pm the next business day. Please check with your pharmacy before picking up your medication.
  • Insurance benefits - We at DHC and the Endoscopy Center do as much as possible to provide you, the patient, with appointments and procedure scheduling, as needed. However, it is impossible to obtain knowledge of every plan, network and specifics of your insurance coverage. It is your responsibility, as the subscriber, to know what your insurance covers and where your services can be scheduled.
  • Our staff will call the refill to the pharmacy before 5 p.m. the next business day. Please check with your pharmacy before picking up your medication.
